DQ1: Communication and Problem Solving

As the manager of an acute care nursing unit, you are approached by Jacklyn Smith, MD and Matthew Gonzalez, NP. They express frustration because the morning vital signs are often not completed by the time they arrive on the unit. You tell them you will look into it and see what you can do to facilitate a more efficient process. After investigating, you find the following:

An RN must take the initial set of vital signs on each shift in your facility per policy. An aide can take subsequent vital signs.

Vital signs are to be taken minimally every 8 hours.

The shift changes at 0700 and the RNs are in report until 0730.

The health care providers arrive at approximately 0730 and usually leave by 0830.

Aides change shift at 0700, and use the last 30 minutes of their shift to complete their documentation.

Using the five part problem solving process from your readings, discuss how you will address this issue so the health care providers get the information they need to make patient care decisions. Discuss how you will modify the process, while still observing both policy and practices on your unit. Analyze what communication strategies would be most effective in implementing a change that is sustainable.
