ETHICAL DILEMMA ONESuzanne has worked in a primary care counselling agency for four years and has become good friendswith a colleague there, Michelle. Michelle is a highly respected and liked member of the team. Suzannediscovered recently that Michelle has been drinking heavily, and for some time. While she appears to besober at work, Suzanne is fearful for Michelle’s clients and her capacity to work professionally. Michellebecame distressed when asked about this by Suzanne, and begged Suzanne not to say anything to hermanager – she would ‘lose [her] job and become unemployable’. Suzanne feels torn between trying tosupport her friend to turn things around personally, and the wellbeing of Michelle’s clients, whichincreasingly seems to be compromised. She initially made contact with you on the recommendation of an infertilitysupport group she was attending. She and her partner had been trying for children but have been told thatin all likelihood this is not going to be successful. The counselling appeared to be progressing well and Joywas beginning the difficult process of talking about her grief. The last time you met with Joy she wasangry and distressed. She had noticed a photograph of you and your children on the wall. She felt that thiswas an inappropriate self-disclosure and did not feel able to talk to you anymore. She walked out of thesession half-way through. ETHICAL DILEMMA THREEDr. Vanessa Ives works in a solo private practice. She has been working with Mr. Dorian Gray for severalmonths for signs and symptoms of depression. Mr. Gray comes to some sessions as emotionally intense,and high strung. Dr. Ives has considered the possibility that Mr. Gray suffers with some type of cyclicmood disorder. As part of treatment, Mr. Gray admitted to experiencing anger management issues, to thepoint where he described physically intimidating his wife and pushing her down. They worked on angermanagement skills. Mr. Gray reported progress in this area.Dr. Ives receives a phone message from Mr. Gray’s wife. In the voicemail, Mrs. Gray reports that Mr.Gray has become more physically intimidating and has starting to push her around. The voicemailindicated he has not cause her any significant harm. She requested a session to see Dr. Ives to explainwhat is happening between them. Dr. Ives only met Mrs. Gray informally while she sat in the waitingroom before and after several sessions. Dr. Ives wants to be helpful, but she is struggling with whether sheshould even return Mrs. Gray’s phone call. Dr. Ives has a personal history of being involved in aphysically abusive relationship herself and is concerned about both the clinical and ethical issues involvedregarding calling Mrs. Gray back.
