M.Z. is a 38-year-old female with a past medical history of uterine fibroids and had a myomectomy. It is post-operation day two, and, M.Z.’s temperature is 101.9°F, HR 100, RR 22, BP 100/68, purulent drainage is noted from the wound. The following prescriptions are entered:

1L of 0.9% sodium chloride IV over 2 hours, then continue 0.9% sodium chloride at 125ml/hr.
Obtain blood cultures x2, CBC, and abdominal wound cultures
Acetaminophen 650 mg PO every 4 hours for a temp. > 100.5
Cefepime 2g IV every 8 hours.
Metronidazole 500mg IV every 8 hours
Vancomycin 25mg/kg IV now, then 15mg/kg every 8 hours. Pharmacists to monitor and dose.
