In this patient-centered final project you will provide Tina with education about effective self-care to prevent readmission. Remember to use accessible, professional language demonstrating caring behaviors with use of therapeutic communication as you anticipate any questions or concerns Tina might have. Be sure to include specific tools and resources (such as apps, support groups, handouts, credible web sources) to support health promotion and the management of her care.
